How to Apply for EDX Laboratory Accreditation

The application is completed online. Your work saves as you go, several colleagues can work on it together, and you can stop and come back at any time. This guide walks through each step and lists what to have ready before you start.

Before you start
  • Membership. You do not need to be a member to apply. If you are an APTA / EDX SIG member, there is a place on the Contact Information page to enter your member number; once the Academy confirms it, the member rate is applied to your fee.
  • Gather your documents. Most pages ask for policies and supporting documents — see the document checklist below. Files can be PDF, Word, JPG or PNG, up to 25 MB each.
  • Sample reports must be de-identified. Remove every patient name, date of birth, record number and any other identifying detail before uploading. You will be asked to confirm this when you submit.
  • Payment is online only — by card or US bank account (ACH). Every page is open from the start; the fee and, for members, confirmation of your member number are needed before you can submit.

Application fee

Laboratory sizeMemberNon-member
Main laboratory, 1–3 providers, up to 3 satellite facilities$475$950
Main laboratory, 1–3 providers, up to 10 satellite facilities$650$1,295
Main laboratory, 1–3 providers, over 10 satellite facilities$795$1,500
Each additional provider (over 3)$75$150

The site works out your fee from the clinics and licensed providers you enter. Fees are non-refundable. If you add clinics or providers after paying and the fee goes up, the difference is due before you can submit.

Create your account
  1. Go to the application page and choose Create account.
  2. Enter an e-mail address you check regularly and a password of at least 8 characters.
  3. Open the confirmation e-mail we send you and click the link to activate your account, then sign in.

Forgot your password? Use “Forgot password?” on the sign-in screen and we will e-mail you a reset link.

Contact information and your team
  • Contact Information — the person completing the application (this can be an administrator or assistant), the laboratory's name, phone, e-mail and mailing address, and your APTA / EDX SIG member number if you have one.
  • Team — optional. Invite colleagues by e-mail to help fill out the application. They create their own account with that e-mail address and see the same application. If two people edit the same section, the site warns before anything is overwritten.
Clinics and providers
  • Clinics/Contracts — your main laboratory and every satellite clinic, marked as private or contract.
  • Provider Information — the laboratory director, electromyographers and NCS technicians, each listed once even if they work at several clinics. For each licensed provider, say whether they are Board Certified, Board Eligible or a Resident. If an electromyographer or NCS technician is a resident, give the name of the ABPTRFE program they are enrolled in.

Save each page before moving on — these lists set your fee and decide which pages and documents you will see later.

Pay the application fee
  • Open Payment to see your fee and choose Pay online. You will go to Stripe's secure payment page and can pay by card or US bank account.
  • You can pay at any point before you submit, and keep working on the other pages either way.
  • Card payments confirm immediately. Bank (ACH) payments take about four business days to clear and show as paid automatically.
  • A receipt is e-mailed to you, and every payment appears in the payment history on this page.
Laboratory pages (for each clinic)

Each of these pages is completed separately for every clinic — choose the clinic at the top of the page.

  • Laboratory — accessibility, rooms, and fire, power outage, medical emergency and universal precautions policies (A1–A6).
  • Lab Policies — state practice act, technician and non-board-certified provider supervision, job descriptions and competency documents (P1–P3).
  • Performance — study counts and case mix (enter the total patients seen and the percentage with each condition; the number of patients is calculated for you), procedural handbook, normal values and outcome monitoring (B1–B6).
  • Equipment & Supplies — each EMG/NCS machine with manufacturer, model, serial number, in-service date and last safety check, plus electrical safety and supply policies (C1–C3).
  • Medical Records & Privacy — records retention and HIPAA policies (D1–D2).
Personnel pages
  • Credentials — one card per person: experience, licences by state, certifications, CPR, continuing education, the clinics where they test, and their documents (E1–E10). Sample reports (E7–E9) are needed only for providers who are not board certified.
  • Studies — for each provider, the total patients seen in the last 12 months and the percentage with each condition (Appendix F-1; the number of patients is calculated for you), plus their study counts (Appendix F-3).
Review and submit
  • Review & Submit shows how complete each section is and lists anything still missing, with a link straight to the page that needs it. The progress bar at the top of every page shows the overall percentage.
  • When everything is complete, confirm that all sample reports are de-identified and select Submit.
  • You will receive a confirmation e-mail. After submitting, the application is locked for editing.
After you submit
  1. Administrative check. Academy staff confirm the application is complete and the fee is paid. If something is missing, the application is returned to you with a note explaining what to fix; make the changes and submit again.
  2. Board review. A member of the Accreditation Board reviews the application against the accreditation criteria and grading rubric.
  3. Decision. The Academy notifies you of the outcome by e-mail.

You can sign in at any time to see your application's status.

Confidentiality and review of your application
  • Confidentiality agreements. Everyone who reviews applications — Academy program staff and the members of the EDX Laboratory Accreditation Board — has signed a confidentiality (non-disclosure) agreement.
  • Your data is protected. Your application and documents are stored on a secure, access-controlled system. Only the Academy's program administrators and the Board member assigned to your application can see them, and they are used only to review your application for accreditation.
  • AI-assisted initial review. The first pass of the review uses an AI assistant (Claude, by Anthropic). It reads your answers and your policy documents and suggests a preliminary grade against the grading rubric. It is used through a secure business service that does not use your information to train AI models, and only for this review. Sample patient reports and personal documents (CVs, licenses, CPR cards) are never sent to the AI.
  • People make the decision. The assigned Board member checks every AI suggestion — confirming, changing or discarding it — and the accreditation decision is made by the Board.

You will be asked to accept this on the Review & Submit page before you submit.
